If you sustain an injury, your team manager will need to complete an Accident Report Card at the time of the incident. This card details the match played, where the incident occurred with date, clubs involved, ground, as well as the incident itself. The incident, if serious, should also be noted on the team sheet by the team manager. The team sheet is handed into the Home Ground canteen and the Accident Report Card is taken back to Rovers clubhouse and given into the President who will pass it onto BDAFA.
An insurance claim form may be needed to be completed.
This will need to be completed by the injured player, employer, if claiming for loss of income and the doctor and then handed back to Rovers President to hand onto BDAFA, who will pass it onto Soccer NSW and then onto the insurance company. The insurance company will then deal direct with injured party directly.
